氟比洛芬酯聯(lián)合舒芬太尼用于腹腔鏡全宮切除術(shù)術(shù)后鎮(zhèn)痛的效果

　　[摘要]目的 探討氟比洛芬酯聯(lián)合舒芬太尼用于腹腔鏡全宮切除術(shù)術(shù)后鎮(zhèn)痛的效果。方法 選取我院2015年5月～2016年7月行腹腔鏡全宮切除術(shù)患者200例，隨機(jī)分為研究組與對(duì)照組，每組100例。對(duì)照組患者術(shù)后給予舒芬太尼、氟哌利多鎮(zhèn)痛，研究組患者在對(duì)照組基礎(chǔ)上加用氟比洛芬酯鎮(zhèn)痛。采用視覺模擬評(píng)分（VAS）對(duì)兩組患者術(shù)后1、4、8、16、24、48 h鎮(zhèn)痛情況進(jìn)行評(píng)分，并觀察不良反應(yīng)發(fā)生情況。結(jié)果 研究組患者術(shù)后1、4、8、16、24、48 h的VAS評(píng)分顯著低于對(duì)照組，差異有統(tǒng)計(jì)學(xué)意義（P<0.05）。研究組患者的頭暈、皮膚瘙癢、呼吸抑制、惡心嘔吐和消化道潰瘍等不良反應(yīng)發(fā)生率顯著低于對(duì)照組，差異有統(tǒng)計(jì)學(xué)意義（P<0.05）。結(jié)論 氟比洛芬酯聯(lián)合舒芬太尼用于腹腔鏡全宮切除術(shù)術(shù)后鎮(zhèn)痛效果顯著，不良反應(yīng)少，值得臨床推廣應(yīng)用。

　　[Abstract]Objective To investigate the effect of Flurbiprofen Axetil combined with Sufentanil on postoperative analgesia in laparoscopic total hysterectomy.Methods A total of 200 patients with laparoscopic total hysterectomy in our hospital from May 2015 to July 2016 were selected as research objects.All of the patients were randomly divided into the study group and the control group，with 100 cases in each group.Patients in the control group were given Sufentanil and Droperidol analgesia，and the patients in the study group were treated with Flurbiprofen Axetil on the basis of the control group.The visual analogue scale （VAS） of the two groups were evaluated at 1，4，8，16，24，48 h，and the adverse reactions were observed.Results The VAS scores of the study group at 1，4，8，16，24，48 h were significantly lower than those in the control group，with significant difference （P<0.05）.The incidence of adverse reactions such as dizziness，pruritus，respiratory depression，nausea，vomiting and gastrointestinal ulcer in the study group were significantly lower than those in the control group，the difference was statistically significant （P<0.05）.Conclusion Flurbiprofen Axetil combined with Sufentanil for postoperative analgesia in laparoscopic total hysterectomy is effective and less adverse reactions，so it is worthy of clinical promotion and application.

　　隨著環(huán)境及生活方式的改變，婦科疾病發(fā)病率呈逐步上升趨勢(shì)，患者因患子宮肌瘤、子宮內(nèi)膜增生等疾病，需進(jìn)行全子宮切除治療[1]。目前，臨床常采用腹腔鏡全宮切除術(shù)治療，該術(shù)式具有腹部傷口小、住院時(shí)間短等優(yōu)點(diǎn)，但因患者全子宮切除，手術(shù)創(chuàng)傷大，術(shù)后疼痛劇烈，嚴(yán)重影響患者生活質(zhì)量，因此良好的術(shù)后鎮(zhèn)痛效果對(duì)患者具有重要意義。臨床常采用舒芬太尼鎮(zhèn)痛，該藥具有腸溶性強(qiáng)，起效快等特點(diǎn)，但舒芬太尼作用于中樞神經(jīng)系統(tǒng)，易積蓄，不良反應(yīng)較多，長(zhǎng)期使用可致上癮[2]。研究顯示，氟比洛芬酯具有良好的鎮(zhèn)痛效果，不良反應(yīng)少，術(shù)后可立即使用[3]。本研究選取我院行腹腔鏡全宮切除術(shù)患者作為研究對(duì)象，探討氟比洛芬酯聯(lián)合舒芬太尼用于術(shù)后鎮(zhèn)痛的效果，現(xiàn)將結(jié)果報(bào)道如下。
