孕11~14周胎兒超聲軟指標在胎兒心臟結構評估中的價值

          【摘要】 目的:對孕11~14周胎兒超聲軟指標在胎兒心臟結構評估中的價值予以探討。方法:隨機選取筆者所在醫(yī)院2016年1月-2017年1月開展超聲檢查的孕11~14周胎兒2 000例,對其檢查結果及妊娠結局進行觀察記錄,分析超聲軟指標在早孕期胎兒先天性心臟病中的診斷情況。結果:所選2 000例胎兒中,通過開展產(chǎn)前超聲診斷,發(fā)現(xiàn)15例嚴重先天性心臟病,檢出率為0.45%,這其中有6例存在染色體異,F(xiàn)象,其中有4例合并有三尖瓣反流(TR),有3例合并靜脈導管血流a波(DVa)倒置,胎兒頸后透明層厚度(NT厚度)值大于第99百分位的18例胎兒中有5例診斷為先天性心臟;NT厚度值小于第95百分位的1 879例胎兒中,有8例診斷為先天性心臟;NT厚度值處于95~99百分位的103例胎兒中有2例診斷為先天性心臟。結論:為孕11~14周胎兒開展超聲檢查,DVa倒置、TR、NT增厚等軟指標在其心臟結構評估當中具有非常重要的臨床價值,值得在早孕檢查中推廣。

          【Abstract】 Objective:To discuss the value of fetal ultrasonography in the evaluation of fetal heart structure in 11-14 weeks of gestation.Method:From January 2016 to January 2017,2 000 cases who carried out ultrasound in the 11 to 14 weeks of pregnancy to fetuses were randomly selected,the results of the examination and pregnancy outcome were observed records,and then the ultrasound index in early pregnancy fetal heart structure in the false positive rate of the calculation,as the basis for judging the analysis of ultrasound soft indicators in early pregnancy fetal congenital heart disease in the diagnosis.Result:Among the 2 000 fetuses,15 cases of severe congenital heart disease were detected by prenatal ultrasound.The detection rate was 0.45%.There were 6 cases of chromosomal abnormalities.Among them,4 cases had tricuspid valve TR,3 patients with juvenile heart disease were diagnosed fetal caliper(DVa) inverted,5 patients of 18 cases were diagnosed juvenile heart disease by fetal neck transparent layer thickness(NT thickness) greater than the 99th percentile.The thickness of less than the 95th percentile of the 1 879 cases of the fetus,8 cases diagnosed with congenital heart disease,NT thickness of 95 to 99 percentile of 103 cases of fetal in 2 cases diagnosed with congenital heart disease.Conclusion:For the 11 to 14 weeks of pregnancy to carry out ultrasound,DVa inversion,TR,NT thickening and other soft indicators in the evaluation of its heart structure which has a very important clinical and value,it is worth to promote in early pregnancy examination.

          胎兒早期最常見的先天缺陷就是大血管畸形及心臟畸形,這也是導致新生兒死亡的一個非常重要的因素,在早孕期檢查過程中最為常用的一項檢查方法就是超聲檢查,但是常規(guī)的產(chǎn)前超聲篩查在嚴重先天性心臟病方面的檢出率不高,隨著各項研究的不斷深入,有研究發(fā)現(xiàn),一些常見的超聲軟指標在胎兒心臟結構評估工作中發(fā)揮著重要的作用,其中一個典型的超聲軟指標就是NT增厚,其在胎兒嚴重心臟缺陷的篩查過程中發(fā)揮著非常重要的作用,此外,DVa、TR也是胎兒心臟缺陷的高危指標[1]。探究超聲軟指標在胎兒心臟結構評估中的價值具有非常重要的意義,本文就主要針對此予以簡單分析,現(xiàn)將結果報告如下。
