術前介入栓塞在腎臟巨大腫瘤手術中的應用及臨床評價

　　【摘要】 目的：對術前介入栓塞在腎臟巨大腫瘤手術中的應用及臨床效果進行深入分析和評價研究。方法：收集本院2015年8月-2017年8月收治的60例腎臟巨大腫瘤患者作為研究對象，按照隨機數(shù)字表法分為A組（30例，直接進行手術治療）和B組（30例，先進行介入栓塞治療再進行手術治療），對比兩組患者的手術時間、術中出血量、住院時間以及并發(fā)癥發(fā)生情況等指標。結果：B組患者的手術時間（95.25±16.27）min明顯小于A組患者的（150.66±21.44）min，B組患者的術中出血量（195.41±12.43）mL明顯小于A組患者的（640.83±11.04）mL，B組患者的住院時間（2.52±0.73）d明顯小于A組患者的（7.64±0.87）d，兩組比較差異均有統(tǒng)計學意義（P<0.05）。治療后，A組患者治療后出現(xiàn)腰痛2例，出血2例，感染3例；B組患者治療后出現(xiàn)腰痛1例，B組并發(fā)癥發(fā)生率3.33%明顯低于A組患者的23.33%，比較差異有統(tǒng)計學意義（P<0.05）。結論：術前介入栓塞在腎臟巨大腫瘤手術中有良好的應用效果，通過術前介入栓塞，能夠有效縮短腎臟巨大腫瘤手術時間和患者的住院時間，減少術中出血量以及相關并發(fā)癥的發(fā)生，對于提高腎臟巨大腫瘤患者的手術成功率，促進患者后續(xù)的身體恢復有十分積極的作用。

　　【Abstract】 Objective：To analyze and evaluate the application and clinical effect of preoperative embolotherapy in the operation of huge renal tumor.Method：A total of 60 cases which were diagnosed as huge renal tumor in our hospital from August 2015 to August 2017 randomly divided into A group（30 cases，direct surgical treatment）and B group （30 cases，the first preoperative embolotherapy and surgical treatment）according to table of random numbers.The operation time，blood loss，hospitalization time and complications were compared between two groups.Result：The operative time in B group was（95.25±16.27）min significantly lower than that in group A （150.66±21.44）min， the amount of bleeding patients in B group was（195.41±12.43）mL significantly lower than that in group A （640.83±11.04）mL，hospitalization time in the B group was（2.52±0.73）d less than that of A group （7.64±0.87）d，the difference was statistically significant（P<0.05）.In A group，there were 2 cases of lumbago after treatment，2 cases of bleeding and 3 cases of infection；1 case of lumbago in B group after treatment，the complications rate of B group was 3.33% significantly lower than that of A group 23.33%，the difference was statistically significant（P<0.05）.Conclusion：Preoperative embolotherapy has good application effect in huge renal tumor surgery.Preoperative embolotherapy in the operation of huge renal tumor can effectively shorten the operation time，reduce blood loss and related complications，improve surgery success rate，promote thephysical recovery of patients.
