經(jīng)陰道彩色多普勒超聲在診斷剖宮產(chǎn)子宮疤痕妊娠中的價值研究

　　摘   要：目的  觀察在剖宮產(chǎn)子宮疤痕妊娠中應(yīng)用經(jīng)陰道彩色多普勒超聲的診斷價值。方法  選取2015年2月～2017年6月在我院就診治療的剖宮產(chǎn)子宮疤痕妊娠患者120例，均經(jīng)臨床病理學(xué)確診，所有患者全部行經(jīng)陰道和腹部彩色多普勒超聲檢查，并記為觀察組（陰道超聲組）和對照組（腹部超聲組），觀察并對比兩組的檢查診斷結(jié)果和聲像學(xué)分析結(jié)果。結(jié)果  觀察組檢查準(zhǔn)確率為93.33%，高于對照組的81.67%，差異具有統(tǒng)計(jì)學(xué)意義（P<0.05）;觀察組患者中切口處孕囊型26例，包塊型21例，混合型9例，對照組患者中切口處孕囊型25例，包塊型19例，混合型5例，兩組間相比較，差異無統(tǒng)計(jì)學(xué)意義（P>0.05）。結(jié)論  在剖宮產(chǎn)子宮疤痕妊娠中應(yīng)用經(jīng)陰道彩色多普勒超聲進(jìn)行診斷，具有更高的準(zhǔn)確性，對于臨床確診有更佳的意義，值得推廣應(yīng)用。

　　The Value of Transvaginal Color Doppler Ultrasound in the Diagnosis of Cesarean Scar Pregnancy

　　Abstract：Objective  To observe the diagnostic value of transvaginal color Doppler ultrasound in cesarean section uterine scar pregnancy.Methods  120 cases of cesarean section uterine scar pregnancy treated in our hospital from February 2015 to June 2017 were selected.All were confirmed by clinicopathology and all patients were examined by transvaginal and abdominal color Doppler ultrasound and recorded as observation group（vaginal ultrasound group）and control group（abdominal ultrasound group）.The results of diagnostic examination and sonographic analysis were observed and compared between the two groups.Results  The accuracy of examination in the observation group was 93.33%，which was higher than that in the control group 81.67%，the difference was statistically significant（P<0.05）.In the observation group，there were 26 cases of gestational sac type at incision，21 cases of mass type，9 cases of mixed type，in the control group，there were 25 cases of gestational sac type at incision，19 cases of mass type and 5 cases of mixed type.There was no significant difference between the two groups（P>0.05）.Conclusion  Transvaginal color Doppler ultrasound is more accurate in the diagnosis of uterine scar pregnancy during cesarean section.It has better significance for clinical diagnosis and is worth popularizing.

　　剖宮產(chǎn)子宮瘢痕妊娠是一種少見且危險性極大的異位妊娠，指在剖宮產(chǎn)后女性再次妊娠時，孕囊在子宮瘢痕處著床[1]。子宮內(nèi)膜和子宮肌層會在剖宮產(chǎn)的瘢痕位置存在缺陷，隨著胚胎的生長絨毛深入內(nèi)部，侵蝕局部血管，若不及時正確處理，極有可能引發(fā)孕婦大出血，甚至出現(xiàn)子宮破裂，嚴(yán)重威脅患者的生命安全[2，3]。本次研究選取我院120例經(jīng)過病理學(xué)確診為剖宮產(chǎn)子宮疤痕妊娠的患者，分析陰道彩色多普勒超聲對于該病的診斷價值和效果，現(xiàn)將結(jié)果整理報道如下。
